Health care delivery drives only 20 percent of health outcomes.  To truly have an impact on your population’s health you must address the behavioral and social determinants of health - including income, living environment, employment status, and access to transportation and healthy food. This workshop will give you best practices and strategies to identify partners which serve older adults as well as tips on how to engage them and sustain those partnerships.
