HOTEL ACCOMMODATIONS:(Required)
Hospital attendees will be provided with hotel accommodations as required to attend the sessions. Limited to two rooms per night per hospital, maximum of two nights stay. Please note that if a room is reserved for you and you do not show up, your hospital will be billed for the 'no show' room. Thank you!
